Register as a Vendor

To register as a vendor with the City of North Port, the following forms must be completed and submitted to Purchasing for review:

Vendor Registration Sheet

W-9 Form

Additional items may be required for processing including:

• General Liability, Commercial Auto, Workers' Compensation Insurance Certificates
• Proof of Professional Liability (E&O) Insurance Certificate
• Signed Hold Harmless / Indemnification Form
